Client Therapy Portal Transition Form

Hi! My practice is transitioning to a new therapy platform. I am moving from Grow Therapy to TherapyNotes. Please complete the form below so my billing team can move your information and ensure your sessions continue without interruption. 

Once my billing team has received this form, you will also receive an email from TherapyPortal.com with the subject line: Client Portal Invitation. That email will give you the opportunity to create your client portal account for the TherapyNotes platform.
